Job Description
A Health Advisor role is available within a clinical operations team focused on improving patient outcomes through coordinated care. This position supports at-risk patients by ensuring smooth transitions of care, closing treatment gaps, and enhancing overall healthcare delivery through effective communication and follow-up.
Responsibilities
- Conduct post-discharge follow-up calls to assess patient needs and reinforce care plans
- Review and reconcile discharge medications, escalating concerns to primary care providers when necessary
- Coordinate and schedule follow-up appointments with primary care providers and specialists
- Identify high-risk patients and refer them to appropriate care management teams
- Review emergency department discharges and facilitate proper follow-up care
- Address patients’ social determinants of health by connecting them with relevant resources
- Support the completion of open care gaps to improve patient outcomes
- Maintain compliance with HIPAA regulations and patient confidentiality standards
- Demonstrate strong medical knowledge and the ability to handle multiple tasks effectively
Requirements
- Certification as an Emergency Medical Technician (EMT) or Medical Assistant (MA)
- 3–5 years of relevant clinical experience preferred
- Experience in an outpatient healthcare setting is an advantage
- Familiarity with electronic medical records (EMR) systems
- Strong communication and patient engagement skills, especially via phone
- Ability to navigate multiple technology platforms efficiently
- High attention to detail and strong organizational skills
Salary
- $20.00 – $23.80 per hour (excluding bonuses and benefits)
- Eligible for performance-based annual bonus
